Meeting Minutes Template: Blank Form, Board, and Client
Meeting minutes template
A meeting minutes template is a fixed set of headings you fill while the meeting is still fresh: date, people, agenda, a short note per topic, decisions, action items with an owner and a due date, and whatever stayed open. Someone who missed the hour should be able to act from the page without replaying the conversation.
Use the blank form below for an ordinary working meeting. The three filled write-ups are made-up examples, with the same shape for a weekly team sync, a Swiss board (Verwaltungsrat), and a client call. A transcript is a different document. It keeps the words. The minutes keep what changed.

One decision per line, one action per line. If a line has no owner and no date, it is not an action yet. Park it under open questions until both exist.
The minutes sheet is the same layout in the browser, with starters for a team sync, a client call, and a board sitting. It does not record the room and it does not upload a file.
Weekly team sync
A made-up example. Roles only, no real company.
- Title: Product team weekly sync
- When: 6 October 2026, 09:00–09:30 CEST
- Attendees: product lead, engineering lead, design, support
Agenda: release status (10 min), support themes (10 min), next sprint (10 min).
Discussion:
- Release 2.4 is on schedule. One bug in the billing export still blocks the build.
- Support is seeing the same SSO setup questions again. A written guide is in progress.
- Sprint goal: close the export bug and publish the SSO guide. Mobile polish waits.
Decisions:
| # | Decision | Owner | Effective date |
|---|---|---|---|
| 1 | Ship 2.4 only after the export bug is closed | Engineering lead | 8 October 2026 |
| 2 | Mobile polish moves to the following sprint | Product lead | 6 October 2026 |
Action items:
| # | Action | Owner | Due date | Status |
|---|---|---|---|---|
| 1 | Patch the billing export and post release notes | Engineering lead | 8 October 2026 | Open |
| 2 | Publish the SSO setup guide | Design | 9 October 2026 | Open |
| 3 | Send the weekly support summary to the product channel | Support | 7 October 2026 | Open |
Open: Is the written SSO guide enough, or do we still need a short demo? Revisit next week.
Send this the same day. A week later, “we should look at billing” is no longer a task anyone can pick up.
Swiss board meeting
A made-up example of a Verwaltungsrat sitting for a Swiss company limited by shares (AG).
- Title: Board, Q4 planning
- When: 6 October 2026, 14:00–16:00 CEST
- Place: Zug, with a hybrid link for people attending as observers
- Attendees: chair, three board members, CEO, CFO as a guest for the finance section
Agenda: approval of the previous minutes, CFO report, 2027 budget, risk register, closed items if needed.
Discussion:
- The previous minutes were approved with no correction.
- The CFO presented cash runway and the hiring plan. The board asked for a scenario in which the enterprise deal slips.
- Two vendor-concentration items on the risk register need a named owner.
Decisions:
| # | Decision | Owner | Effective date |
|---|---|---|---|
| 1 | Previous minutes accepted as written | Chair | 6 October 2026 |
| 2 | 2027 budget draft approved for circulation to shareholders | CEO | 20 October 2026 |
| 3 | Enterprise-deal scenario added to the next CFO report | CFO | 3 November 2026 |
Action items:
| # | Action | Owner | Due date | Status |
|---|---|---|---|---|
| 1 | Circulate the approved budget pack to shareholders | CEO | 20 October 2026 | Open |
| 2 | Update the risk register with the vendor mitigations | CFO | 27 October 2026 | Open |
Open: Closed-session items stay in a separate note. They are not repeated in the minutes that go to a wider circle.
For an AG, Art. 713 para. 3 of the Code of Obligations requires minutes of the board’s deliberations and resolutions, signed by the chair and the minute-taker. That signature line is part of the record. Missing minutes do not by themselves void a resolution, as the Federal Supreme Court held in BGE 133 III 77. The duty to keep minutes still stands. This paragraph is orientation, not legal advice.
A weekly team sync does not inherit that signature rule. Do not turn every standup into a board pack. Still name the minute-taker, because someone has to own the draft.
Read the decisions aloud before you close the item. The minute-taker writes the sentence the chair just read, not a reconstruction from memory on the train home.
Client call
A made-up example. The client is unnamed on purpose.
- Title: Phase 2 scope review
- When: 6 October 2026, 11:00–11:45 CEST
- Attendees: client project lead, your lead consultant, your delivery manager
Agenda: phase 1 recap, phase 2 scope, timeline and how you will communicate.
Discussion:
- Phase 1 deliverables were accepted, with one documentation tweak still open.
- Phase 2 adds a reporting module. The client wants it rolled out in stages.
- You agreed a weekly status email and one shared action list.
Decisions:
| # | Decision | Owner | Effective date |
|---|---|---|---|
| 1 | Phase 2 starts on 20 October 2026 | Client project lead | 6 October 2026 |
| 2 | The reporting module ships in two milestones | Your lead consultant | 6 October 2026 |
Action items:
| # | Action | Owner | Due date | Status |
|---|---|---|---|---|
| 1 | Send the updated statement of work and milestone dates | Your lead consultant | 8 October 2026 | Open |
| 2 | Client shares sample report layouts | Client project lead | 10 October 2026 | Open |
| 3 | Book the weekly status slot | Delivery manager | 7 October 2026 | Open |
Open: Which SSO identity provider the client will use in production. Their IT confirms.
Client minutes leave out internal nicknames, staffing gripes, and anything the contract treats as confidential. Mark those lines before you send. If you are unsure, send the decisions and the action table, and keep the discussion summary internal until the lead has read it.
How to write a decision line
A decision line is one sentence a person can quote later. It names the outcome, who owns it, and when it takes effect. “We discussed the release” is a topic. “Ship 2.4 only after the export bug is closed, owner engineering lead, from 8 October” is a decision.
An action item is the work that follows. One decision often creates two or three actions. Each action still needs its own name and date. “The team will look into SSO” fails that test. Move it to open questions, or name a person before you leave the call.
Write the discussion in a few lines per agenda item. Keep exact wording only when the wording is the decision, which is common in a board resolution and rare in a team sync.
Put it in the minutes
- The outcome, in one sentence
- The person who will do the next piece of work, and the date
- A question that stayed open, including who brings it back
Leave it out
- A play-by-play of who argued what
- Jokes, tone, and side comments
- A task with no owner, written as if it were agreed
Mistakes that waste the next meeting
The minutes are a transcript. Readers search for the decision. A page of dialogue hides it. Keep a transcript only when you need the words, and link it from the minutes instead of pasting it in.
Actions with no owner. The line survives in the doc and dies in the calendar. If you cannot name a person before the meeting ends, the item is still open.
Decisions buried in a paragraph. The table is what you will search next month. The paragraph is context for someone reading today.
No date, no attendees, no next meeting. A note without those three is hard to trust when a client or a board asks what was agreed.
The draft went out as the record. For a client and for a board, the facilitator reads the draft before anyone outside the room sees it. Say “draft” in the title until that happens.
If you are starting from a long Zoom audio file rather than notes, the Zoom transcription tool turns a local recording into text in the browser first. You still copy outcomes into this template yourself.
When the template is the wrong tool
A meeting minutes template does not record the meeting, does not collect consent, and does not set your retention period. You tell people when a recording is running. You store the file where your organization allows. You edit generated text before it is sent.
The blank form also does not know who spoke. Speaker names, if you need them, come from a workflow that separates voices. On a Mac, Vemoir does that on the device with speaker profiles. The free browser tools do not.
Already holding a transcript and want headings rather than a blank page? The meeting notes from transcript tool drafts structure from text you already have. The file is not uploaded. Limits, as of September 2026: files up to 15 minutes, three successful runs a week, no live recording, no speaker labels. Swiss German speech from the public models comes out as Standard German. Longer meetings belong in the Mac app.
How a recording fills the same headings
On a Mac, Vemoir records from the menu bar: the microphone and system audio for Teams, Zoom, Meet, and any other app that plays sound on the machine. No bot joins the participant list. You start and stop each recording. After you stop, local models on the Mac write a transcript and a record with a summary, decisions, action items, and a follow-up draft. Notes you type during the meeting are folded into that record.
That maps onto the template:
- The discussion block comes from the summary, grouped by topic.
- Decisions and action items arrive as lists you edit before anything is shared.
- Export copies only the sections you choose into mail, a PDF, or the wiki. The recording can stay on the Mac. See export and share.
Transcription and summaries run on the Mac unless you opt in to a cloud model with your own API key. Once the models are installed, the workflow can run offline. Internet is for setup and model downloads. macOS asks for Screen Recording permission because that is how the system lets an app hear other apps. Vemoir uses it for meeting audio and does not record the screen.
Want the next meeting to land in these headings without a file hunt? Vemoir records it from the Mac menu bar. Capture details are on capture and models. The minutes workflow is on minutes and record.
Frequently asked questions
What should a meeting minutes template include?
Title, date and time, place or link, facilitator, minute-taker, attendees, apologies, agenda, a short discussion by topic, decisions, action items with owner and due date, open questions, and the next meeting. The blank form above is enough for most working meetings.
How is a decision log different from narrative minutes?
A decision log is the tables: what was agreed, who acts, by when. Narrative minutes add a few lines of context per agenda item. You usually want both, with the tables easy to scan. A full dialogue is a transcript, kept separately when you need it.
How long should meeting minutes be?
For a one-hour working meeting, one or two pages is a healthy result. Board and client minutes run longer when a resolution has to keep its exact wording. The discussion should still be shorter than the transcript.
Who should write the minutes?
Someone who is not also running the discussion, when you have that person. On a small team the facilitator can type decisions into the template live. For a Swiss AG board, Art. 713 para. 3 CO expects the chair and the minute-taker to sign.
Are minutes the same as meeting notes?
Notes are what you jot for yourself. Minutes are the shared record of outcomes. You can start from notes and promote the lines that have an owner and a date. The rest stays in your notebook.
Can I use this for a Verwaltungsrat?
Yes, with the legal form added: deliberations and resolutions in the minutes, signed by the chair and the minute-taker, closed items kept out of the shared copy. The board example above shows the tone. It does not replace your articles of association or legal advice.
Does Vemoir replace the template?
It fills the same sections from a recording on your Mac. You still edit, remove sensitive lines, and decide what is shared. The template is the shape of the record, and the app is one way to draft it. See minutes and record.
Can the browser tool write minutes from a transcript?
The meeting notes from transcript tool drafts notes from text you already have, without uploading the file. It does not record a live meeting and it does not label speakers. A long or sensitive meeting belongs in the Mac app.
Do I need consent before recording?
That depends on who is on the call and which rules apply to you. Tell people at the start, and stop if someone objects and your policy says to stop. Vemoir does not make a recording lawful. Consent and retention stay with you.
Next step
Paste the blank meeting minutes template into the next agenda, or open the minutes sheet and fill decisions while you are still in the room. If the audio should become the draft, record on the Mac and export only the sections you mean to share. For text you already have, start at meeting notes from transcript.
